Health Outreach Programme
Monthly mobile health camps and home-based outreach bringing antenatal care, child health, immunisation and screening to villages across the parish.

Background
Why this matters.
Even in well-served districts, the furthest households are often the last to receive care. Distance, time off work and transport cost discourage clinic visits — especially for mothers and the elderly.
Vision
What we are building toward.
A reliable monthly outreach calendar covering every village in Kashangura Parish, staffed by clinicians and supported by community birth attendants, with a continuous feedback loop into the Foundation's data dashboard.
Community Need
The needs this addresses.
- Antenatal and postnatal care closer to home
- Catch-up immunisation for missed children
- Screening for hypertension, diabetes and anaemia
- Health education tailored to local languages
- Continuity of care between outreach and referral
Proposed Implementation
How we intend to deliver it.
- 1Map villages, agree calendar with parish leadership and the District Health Office.
- 2Recruit and train an outreach team; equip a mobile kit and transport.
- 3Pilot one quarter; publish results; refine the model.
- 4Scale to monthly coverage with continuous quality improvement.
